Licensing Program Analyst (LPA) Jeremey McClain met with facility representative Mavinder Pannu for an unannounced Case Management Inspection regarding an Unusual Incident Report. LPAs observed nine infants supervised by five staff in separate rooms. It was reported that on 02/25/2026 child 1 was fed milk from another child while in care. Child 1 ingested about 3 ounces of formula milk before another staff informed the staff that was feeding them it was the wrong milk. It was stated that child 1 did not have any reactions from drinking the milk. Parents of child 1 were notified. LPA reviewed child 1’s file, conducted interviews with staff and discussed procedures moving forward with facility representative. A Type B deficiency is cited on the 809-D page of the report. An exit interview was conducted, and the report was reviewed with facility representative Mavinder Pannu. A Notice of Site Visit was provided and shall remain posted for 30 days.
