What the inspector wrote

An unannounced case management inspection was conducted today by Licensing Program Analysts Owens and McClain. LPA's met with Director, Anna Keithler. Present at time of inspection was 79 preschool children with 9 staff. The purpose of the case management inspection is to address an unusual incident that occurred on January 29, 2024 that was self reported to Community Care Licensing by the facility. A child was left in a classroom alone, for approximately 20-30 seconds, during transition from outside to inside. The child was found by a another child's parent. The child was taken to the appropriate classroom to be supervised and unharmed. See 809-D for deficiency

Supervision shall include visual observation. This requirement was not met by: A child was left unattended in a classroom during transition from outside to inside. Child was found by another child's parent and taken to a staff. Child had no injuries. Parent was notified.This is an immediate risk to a child. Civil Penalty assessed.
