What the inspector wrote

Licensing Program Analyst (LPA) Jeremey McClain met with facility representative Anna Galindo for an unannounced Case Management Inspection regarding an Unusual Incident Report. LPAs observed five infants supervised by two staff in one classroom and 14 infants supervised by four staff in another classroom. Facility self-reported that on 08/01/2025 a child (C1) began vomiting two hours after they ingested a dairy product. The food was provided by Staff 1 during snack time. The child\342\200\231s parents were contacted, and the child was immediately picked up and taken to the emergency room. During today\342\200\231s inspection LPA reviewed C1\342\200\231s file, infant needs and services plans, and conducted interviews with facility representative Anna Galindo, as Staff 1 was not present during the inspection. Deficiencies are cited on the 809-D page of this report, as they pose a potential threat to the health and safety of children in care. An exit interview was conducted, and the report was reviewed with facility representative Anna Galindo. A Notice of Site Visit was provided and shall remain posted for 30 days.

This requirement was not met as evidenced by the facility reporting via Unusual Incident Report that on 08/01/2025, C1 was given dairy by Staff 1 during snack time. Facility has documentation via Health History and the Infant Needs and Services plan that the child shall not have dairy. This posed a potentital threat to the health and safety of the child in care.
