What the inspector wrote

On 09/15/2025, Licensing Program Analyst (LPA) Soleil Marx met with Director, Anna Keithler, for the purpose of conducting a case management inspection. LPA observed a census of 18 infants being supervised by 7 staff in two separate classrooms. LPA obtained information from interviews with the Director and staff that revealed there had been 2 or more cases of hand, foot, and mouth disease circulating the infant/toddler classroom. The facility had provided an exposure notice to families, sanitized floors, materials and surfaces, and adhered to their sick policies. LPA reviewed records and did not observe an Unusual Incident Report on file for the outbreak. Director stated they did not notify the department of the epidemic outbreak, because they were not aware that they had to. Not reporting an epidemic outbreak of two or more cases to the department puts the facility out of compliance with reporting requirements. Director submitted the Unusual Incident Report to LPA Marx and Director reviewed reporting requirements. One type B deficiency was cited on the 809-D page. Exit interview conducted and report reviewed with the Director. Appeal Rights. A notice of Site Visit was provided and must remain posted for 30 days.
