What the inspector wrote

On Tuesday, March 3, 2026, Licensing Program Analyst (LPA) Lea Habtom arrived at the facility for a complaint investigation. LPA met with licensee, Ellen Nelson. During today's inspection. there was a census of 13 children consisting of 5 infants, 6 preschool and 2 school age children being supervised by 2 staff and the licensee. All adults present during today's inspection have fingerprint clearances and associations. During the inspection, LPA reviewed children's files and determined that 4 children do not have any licensing forms and immunization records. Due to the missing documents, a case management inspection was created to address the deficiency on a 809-D. This report was reviewed with the licensee, Ellen Nelson. A notice of site visit was provided to be posted for 30 days. Appeal rights were provided.

This requirement was not met as evidenced by 4 children who do not have licensing forms and immunization records during today's inspection which is potential risk to the children in care.

    102416.5(d)(2)(b)Staffing Ratio and Capacity: For a Large Family Child Care Home, the maximum number of children for whom care may be provided at any one time when there is an assistant provider in the home b) No more than three infants are cared for during any time when more than 12 children are being cared for. This requirement was not met as evidenced by an interview with the licensee who admitted to being over the infant ratio and the census during today's inspection which was 5 infants instead of 3. This is an immediate health and safety risk to the children in care.
