Licensing Program Analyst (LPA) Jeremey McClain met with facility representative Michelle Wise for an unannounced Case Management Inspection regarding an Unusual Incident Report. LPAs observed 49 children supervised by six staff members. It was reported that on 04/16/2026, Child 1 sustained an injury while in care. It was stated that Child 1 was leaning on the back of Child 2 and fell over and injured their arm while on the playground. Staff observed and tended to Child 1 after realizing the child was hurt. Child 1’s parent was contacted. Child 1 was later diagnosed with a broken bone in their arm. Child 1 has since returned since the incident. During today’s inspection LPA reviewed Child 1’s file, observed the playground area where the incident occurred, and conducted interview with staff. Child 1 was not available for an interview during today’s inspection. An exit interview was conducted, and the report was reviewed with facility representative Michelle Wise. A Notice of Site Visit was provided and shall remain posted for 30 days.
