Inspector’s narrative

What the inspector wrote

On November 9, 2021 at 1:00 PM, Licensing Program Manager Keven Peters and Licensing Program Analyst Amanda Blesi conducted a scheduled informal meeting with Director, Susan Tucker, Program Compliance Manager, Janis Adkins, and Vice President of Center Operations, Elizabeth Philips. The reason for this informal meeting is to discuss a recent Type A violation for lack of supervision which was cited on 09/17/2021. LPM defined the difference between Non-Compliance and an Informal meeting. LPM advised that the purpose of today's meeting is to help the facility gain compliance. On 08/18/2021, Community Care Licensing office received a complaint alleging staff do not provide adequate supervision to children in care. During the investigation LPA has learned that some children in care were touched in the private parts and some witnessed children exposing their privates while in care at Catalyst Kids - Sierra. Staff were unaware the children were engaging in this behavior. Susan Tucker stated the following steps are taken to correct the citation: - they have altered the restroom use to include smaller groups, students go in one at a time, students no longer use urinals and they will use the stall closest to the door for supervision. -they have supervision modules staff utilize for training purposes. During today's meeting, LPA suggested that licensee review the Department web site www.ccld.ca.gov for updated regulations and important information regarding licensing. LPM suggested that Licensee can view information videos at www.ccld.childcarevideos.org . Exit interview with Susan Tucker
