What the inspector wrote

On December 3, 2025, Licensing Program Analyst (LPA) Stephanie Piring met with Facility Representative,Jessica Anaya, for the purpose of conducting a Case Management Inspection. The purpose of today's inspection was explained. During todays visit, there were no children in care. Sacramento RO received an Unusual Incident Report from the facility self-reporting an absence of supervision incident that occurred on November 19, 2025. During today's inspection, LPA made observations, and interviewed the Center Manager and staff regarding the incident. LPA determined by incident report and interview that the substitute present that day took the children to the restroom across the play yard. The Substitute staff transitioned the children from the bathrooms back to the classroom, leaving a child in the restroom. Once they returned back to the classroom, the staff realized a child was missing and went back to look for the child. The child was found walking back near the gate headed to the classroom. The Facility estimates the child was alone for 1-2 minutes. The Center Manager contacted he parents immediately after the incident. The Facility has put a plan in place putting Teachers in charge of bathroom runs, a bathroom log was implemented, and head count will continued to be utilized upon return. Based on Today’s inspection, One Title 22 deficiency is being cited on 809-D and an immediate civil penalty of $500 is being assessed. LPA Stephanie Piring informed facility representative Jessica Anaya that this report dated 12/03/25 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA Stephanie Piring informed the facility representative to provide a copy of this licensing report dated 12/03/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview was conducted and a copy of this report was given to Facility Representative Jessica Anaya. Notice of site was provided and must remain posted for 30 days. Appeal rights provided.
