What the inspector wrote

Licensing Program Manager (LPM) Mai Lor and Licensing Program Analyst (LPA) Stephanie Piring met with Facility Representatives Jessica Anaya and Nicole Rowe for the purpose of an informal office visit. LPM Lor defined the difference between non-compliance and an informal meeting. LPM advised that the purpose of today's meeting is to help the facility gain compliance. Today's informal meeting was to discuss the type A citation issued on 12/3/25 during a case management inspection. On 12/3/25 the facility was cited a type A citation regarding a self-reporting Absence of supervision. The Facility Representatives stated that they have taken the following steps to maintain compliance: 1. Retraining on Health and Safety 20 Minute Modules Supervision Ratios 2. Rewatch CCLD Supervision Video 3. Placing reminder check points on the fence 4. Use a Bathroom Log at every bathroom break 5.Review and sign Catalyst Supervision Policy 6.Avoid using substitutes for Transition Logs. LPM Lor provided information regarding the Technical Support Program (TSP), which is a non-enforcement arm of the Community Care Licensing Division offering on site support to licensees and providers. This report was reviewed with the Facility Representatives Jessica Anaya and Nicole Rowe
