What the inspector wrote

Licensing Program Analyst (LPA) Jeremey McClain met with licensing representative Angelina Sullivan for unannounced Case Management Inspection regarding an Unusual Injury that was reported on 10/31/2025. LPA observed supervision of 76 preschoolers and 25 toddlers by 14 staff. It was reported that on 10/30/2025, a child suffered a cut near their eyebrow that required medical attention. It was reported that child 1 tripped after stepping down from a step stool which caused a gash. Child 1 was treated immediately by staff and their parents were contacted as well as emergency services. The child was transported to the hospital where they later received stitches. LPA made observations to the classroom and the furniture. LPA conducted a brief interview with staff and Child 1. Staff have removed some furniture in order to prevent the same incident from occurring again. LPA did not observe any apparent risk or hazards. An exit interview was conducted, and this report was reviewed with licensing representative Angelina Sullivan. A Notice of Site Visit was provided and shall remain posted for 30 days.
