Licensing Program Analyst (LPA) Jeremey McClain met with facility representative Marci Pilgrim for an unannounced Case Management Inspection regarding an Unusual Incident Report. LPAs observed 52 children supervised by seven staff members. It was reported that on 3/21/2025, a child stuck a pebble up their nose. The facility contacted the child\342\200\231s parents after they were unable to remove the pebble. The pebble was later removed when the child visited urgent care. It was reported that there were no further injuries or concerns. During today\342\200\231s inspection LPA reviewed the child\342\200\231s file and conducted interviews with staff. LPA and licensing representative discussed measures for prevention moving forward. An exit interview was conducted, and the report was reviewed with facility representative Marci Pilgrim. A Notice of Site Visit was provided and shall remain posted for 30 days.
