What the inspector wrote

Licensing Program Analyst (LPA) Jeremey McClain met with facility representative Marci Pilgrim for an unannounced Case Management Inspection regarding an Unusual Incident Report. LPAs observed 71 children supervised by eight staff members. It was reported that on 9/11/2025, a child suffered an injury to their head that required staples. The child suffered the injury after falling from a small play structure and hitting the back of their head on another structure nearby. Staff immediately tended to the child and contacted their parents. The child was later seen in urgent care. During today\342\200\231s inspection LPA reviewed the child\342\200\231s file and interviewed staff. LPA observed the play area and structures where the incident occurred. No deficiencies were observed. An exit interview was conducted, and the report was reviewed with facility representative Marci Pilgrim. A Notice of Site Visit was provided and shall remain posted for 30 days.
