Follow-up on corrections·April 3, 2024

On April 3, 2024 at 12:40 pm, Licensing Program Analysts (LPAs) Lea Habtom and Dao Vang met with director/owner, Paolo Sarmiento, for an unannounced case management inspection. Upon arrival, LPAs observed 20 preschool and 5 school age children being supervised by the director/owner and 2 staff members. LPAs reviewed staff personnel files and checked Guardian for background clearances. LPAs learned from staff interview that one of the staff has not completed a fingerprint clearance prior to working at the facility. The staff member began working at the facility in February of 2024. The staff member was supervising 10 napping preschool children at the time of arrival, escorted children to the restroom and was supervising children alone at 3:18 pm. A deficiency is being cited based on LPAs observations, interviews conducted and record review completed. This report was reviewed with director/owner Paolo Sarmiento. A notice of site visit was provided to be posted for 30 days. Appeal rights provided. Title 22 Deficiency has been cited on the attached LIC 809-D . LPA Lea Habtom informed owner Paolo Sarmiento that this report dated April 3, 2024 documents 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Lea Habtom informed the owner Paolo Sarmiento to provide a copy of this licensing report dated April 3, 2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Appeal Rights given.
