This requirement was not met as evidenced by an interview with the licensee on 4/11/2022. Licensee stated that there were still families that had not recieved form LIC 9224 regarding a citation issued on 07/2021. This poses a potential threat to the health and safety of children in care.

Personal Rights.(a)The licensee shall ensure that each child is accorded the following personal rights: (1) To be accorded dignity in his/her personal relationships with staff and other persons. This requirement was not met as evidenced by the unusual incident reported on 06/17/2021 by licensee, which stated that Staff #1 reacted to Child #1 misbehavior towards other children by yelling at Child #1 and forcefully moving them in a manner that was inappropriate. If not corrected, this poses an immediate risk to the health and safety of children in care.
