What the inspector wrote

Licensing Program Analyst (LPA) Jeremey McClain met with Catalyst Kids Regional Director Nicole Rowe and Director Kristie Brantley for the purpose of an announced prelicensing. Licensee Representatives are requesting a school-age license to serve 70 school-age children. The program will operate Monday through Friday from 7:00 AM to 6:00 AM. Fire clearance was granted on 07/29/2022. The facility is located at Quarry Trail Elementary School. Licensee Representative acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, menus, and daily schedule. All employee clearances will be associated to Catalyst Kids Sunset Ranch #313620604. The facility will be providing breakfast and afternoon snack and children will bring their own lunches. INDOOR ACTIVITY SPACE: The classrooms used will be 26 and 27. LPA observed a sufficient amount of equipment, tables, and chairs. There is a first aid kit and medications will be stored in classroom. LPA observed cleaning disinfectants are appropriately stored and inaccessible to children. Licensee Representative stated there are no poisons or firearms on the premises. AS and LPA observed water jugs in each of the classrooms. LPA observed functional smoke detectors in the classrooms. LPA observed a paper sign-in/sign-out system. Per Health and Safety Code 1596.806, the facility is exempt from square footage requirement; therefore, LPA did not take measurements. The children will use the school\342\200\231s restrooms, and a separate private restroom for the staff is located in the classroom. Children who become ill during the day will be isolated in the classroom and will use the staff restroom, if necessary. (Page 1/2) OUTDOOR ACTIVITY SPACE: Children will use the elementary playground and structures for outdoor activity. LPA observed a safety label that reads the climbing structure is intended for children from ages 5-12 years old. LPA observed a sufficient amount of equipment. There are no bodies of water on the premises. There are shaded areas supplied by trees. The facility's Plan of Operation is located in the administrative file. Incidental Medical Services and a Plan of Operation is located in the facility file. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/(800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm . LPA discussed the following: supervision; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPA informed Licensee Representatives any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days. This facility evaluation report was reviewed and discussed with Licensee Representatives. Licensee Representative was encouraged to the visit the Department's website at WWW.CDSS.CA.GOV for information regarding childcare updates, forms, regulations and legislation pertaining to child care centers. As of today, the facility will be granted a Provisional License. The follow items must be submitted to LPA by 90 days in order to be granted a regular license: 1. Submit completed LIC 404 2. Proof of Cubbies for Storage 3. Carbon Monoxide Detector (Page 2/2)
