101216.2(e) An aide shall work only under the direct supervision of a teacher.This requirement was not met as evidenced by: Based on observation, interview, and record review, LPA determined that there was a 10 minute period during today's visit in which an aide was left alone with 5 toddlers while the teacher went on break. This poses and immediate health, safety, or personal rights risk to persons in care.
