What the inspector wrote

At 9:05am on 8/20/2025, Licensing Program Analyst (LPA) Matthew Gallo met with facility representative Paolo Sarmiento to conduct a plan of correction visit related to citations issued on 8/8/2025. Upon arrival, LPA observed 5 toddlers supervised by 1 staff and 6 infants supervised by 3 staff. The licensee was previously cited a Type A deficiency on 8/8/2025 for not complying with regulations regarding the physical separation of infants and toddler activity spaces, due to the physical gate dividing the classrooms being broken and removed, and the two groups intermingling. The plan of correction stated that the licensee will fix the gate to ensure that the infant and toddler classrooms are physically separate, will not combine infants and toddlers moving forward, and that LPA will conduct a return visit to ensure compliance. When LPA arrived at the facility, he observed the toddlers to be in the hallway leading to the infant classroom. LPA then observed that the gate providing the physical separation between infants and toddler activity spaces had not been fixed and was still absent, leaving no physical separation between the two spaces. Based on the above facts, the plan of correction was not fulfilled. A civil penalty of $900 is assessed for failure to correct a deficiency (see LIC421FC). The civil penalty will accrue $100 a day until the deficiency is corrected. LPA will conduct a return visit to ensure compliance. Exit interview conducted and report was reviewed with the facility representative, Paolo Sarmiento. A notice of site visit was given and must remain posted for 30 days. LPA provided appeal rights.
