What the inspector wrote

At 3:40pm on 3/5/2026, Licensing Program Analyst (LPA) Matthew Gallo arrived at the facility to conduct a collateral visit. Upon arrival, LPA observed 8 infants supervised by two staff members. LPA visited the facility in connection to observations made during a visit to the separate preschool license at 821 Sterling Parkway. During that visit, LPA observed a fully qualified teacher from this facility leave the premises and conducted a collateral visit to ensure proper ratio is maintained. Based on record review, LPA determined that S1 is a teacher with less than the 12 units required to be fully qualified, and S2 is an aide without units. Title 22 requires a child-teacher ratio of 4 infants to 1 qualified teacher. An aide may be substituted for a teacher in ratio, but only if a fully qualified teacher is directly supervising the children. A Type A Title 22 deficiency is cited on the following LIC809-D LPA Matthew Gallo informed facility representative Paolo Sarmiento that this report dated 3/5/2026 documents 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Gallo informed the licensee to provide a copy of this licensing report dated 3/5/2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the facility representative, Paolo Sarmiento. A notice of site visit was given and must remain posted for 30 days. LPA provided appeal rights.
