What the inspector wrote

At 9:20am on 3/11/2026, Licensing Program Analyst (LPA) Matthew Gallo met with facility representative Paolo Sarmiento to conduct a plan of correction visit related to a 3/5/2026 citation. During today's visit, LPA observed a total census of 8 toddlers supervised by 2 staff and 8 infants supervised by 2 staff. The facility was previously cited a Type A deficiency on 3/5/2026 for not adhering to regulations concerning child-teacher ratio. The plan of correction stated that the facility will reschedule staff schedules so that ratio is always maintained, and that LPA will conduct a return visit to ensure compliance. At 4:45pm during today's visit, LPA observed 6 infants supervised by 2 staff, S1 and S2. Through record review, LPA determined that S1 was an aide without units, and that S2 was a teacher who did not have the 12 units to be fully qualified. The plan of correction has not been fulfilled. A civil penalty of $500 is assessed for failure to correct a deficiency (see LIC421FC). The civil penalty will accrue $100 a day until the deficiency is corrected. LPA will conduct a return visit to ensure compliance. Exit interview conducted and report was reviewed with the facility representative, Paolo Sarmiento. A notice of site visit was given and must remain posted for 30 days. LPA provided appeal rights.
