What the inspector wrote

On February 25, 2026, Licensing Program Analyst Stephanie Piring (LPA) and Licensing Program Manager (LPM) Mai Lor, met with Site Director Rachel Cleveland to conduct an unannounced case management inspection. Upon arrival there were 42 children in care,being supervised by 4 staff across 2 classrooms. Today's case management inspection is to follow up on a self reported incident that occurred on 2/9/26 in which a child sustained an injury requiring medical attention after falling off the monkey bars. During today's inspection, LPA interviewed staff, the child, and observed the play structure. LPA observed the monkey bars located on the school play yard. LPA observed the facilities supervision plan while out on the yard and concluded there was adequate supervision. Site Director states the school site had the structure inspected following the incident, and concluded there were no issues with the structure. No deficiencies were cited during today's inspection. An exit interview was conducted and a notice of site visit was provided and must be posted where visible to parents for 30 days.
