What the inspector wrote

On Thursday, May 7 th , 2026, Licensing Program Analysts (LPAs) Joshua Hatch and Matthew Gallo met with facility representative Bethany Fieguth to conduct an unannounced case management inspection regarding lead testing exceedance. During today's inspection there was a census of 36 children being supervised by 9 staff across all classrooms. The purpose of today's inspection was to create a plan of correction following reports of lead exceedance on two of the facilities water outlets. On 03/03/2026, the facility tested water samples for lead. The lead testing identified two water outlets that have a lead exceedance over the amount of 5.5ppb. The first water outlet sampled is marked A, which has an exceedance of 7.180ppb. The second water outlet sampled is marked B, which has an exceedance of 5.56ppb. Signs will be placed on each outlet in exceedance stating that the outlets should not be used for drinking or food preparation. Bethany Fieguth stated the facility has discontinued the use of the indicated faucets for drinking and food prep and are utilizing delivered bottled water. A deficiency is being cited on the attached LIC809-D page, and is considered a potential health, safety or personal rights risk to persons in care. A notice of site visit was given and must remain posted for 30 days. An exit interview was conducted, and the report was reviewed with Bethany Fieguth. Appeal rights were provided to Bethany Fieguth.
