What the inspector wrote

On 10/28/25 @ 10:14am an unannounced case management inspection was conducted by Licensing Program Analyst (LPA), Erica Laird. LPA Laird met with facility director Casey Nunn in response to an Unusual Incident Report received by the Department on 10/08/25. Per the incident report, on 10/08/25 a child (C1) was riding a scooter on the playground when they stopped suddenly causing them to fall and hit their head on the ground. C1 sustained a bump to the head. The facility immediately called C1's parent who picked C1 up later that day. C1 was taken to the doctor where it was determined the child did not sustain a concussion or any severe injuries. On 10/28/25 LPA Laird conducted an interview with director Casey Nunn. Casey stated C1 was riding an age appropriate scooter on the playground when they abruptly stopped causing their body to fall and hit their head on the ground. Casey stated the child was taken to the doctor and was cleared for any major injuries. Casey stated C1 is back at the facility. Casey stated C1 was not wearing at helmet at the time of injury. LPA Laird reviewed the product guidelines which recommend the use of a helmet while riding scooters. Casey stated the facility will be ordering helmets this week. During today\342\200\231s inspection, the facility was toured and the playground was inspected. There were no deficiencies cited during today\342\200\231s inspection. Exit interview conducted and report was reviewed with director, Casey Nunn. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
