What the inspector wrote

The facility pre-licensing inspection was conducted on 7/30/21 at 10:35 AM via tele-inspection due to the current state of emergency regarding the COVID-19 outbreak by Licensing Program Analyst (LPA), Sandy Husband. The Department received an application for a change of location on 6/23/21 for a preschool license requesting a capacity of 12 preschool children with a toddler component of 12 toddlers for a total capacity request of 24. This is a combination facility with a separate infant license. Operating hours are from 8:00 AM to 4:00 PM, M-F. The preschool facility consists of 1 toddler classroom serving children 18 months to 36 months and 1 preschool classroom serving children 2 years old to entry into 1st Grade. The fire clearance was approved on 6/23/21 for 24 children and was received in the Chico Regional office on 7/19/21. The preschool classroom measured for 14 children and the toddler classroom measured for 18 children. The total indoor capacity measured for 32 children and the outdoor yard area measured for 110 children. Therefore, the facility is approved for the requested capacity of 24 children. Required postings (Parents Personal Rights, Emergency Disaster Plan, Earthquake Preparedness Checklist, etc.) listed on Form LIC 311A are posted. The applicant was advised that all staff are required to have a criminal background clearance on file with Community Care Licensing (CCL). The applicant is aware of the immediate $100 per day civil penalty for anyone providing care or supervision without a criminal record clearance. The Director has been granted an exception as of 7/30/21. The applicant will maintain current on Pediatric CPR and First Aid which expires on 2/13/23 as well as Mandated Reporter Training which expires 1/6/23. The applicant has a valid Preventive Health Practices course and will be taking the lead training on 7/31/21. Children will use the fenced outdoor play area. Sign-In/Out procedures were reviewed. The applicant/director understands that if a manual Sign-In/Out procedure is used, full legal signatures are required. Cubbies will be available for children to use. There are separate entrances for each classroom and health checks will be conducted. There are pull fire alarms, a working carbon monoxide detector and charged fire extinguisher in the facility rated at least (Continued on LIC 809-C) (Continued from LIC 809) 2A10BC. All Exits are marked. Poisons will be locked in each classroom cabinet. There are two bathrooms with a total of 3 toilets and 3 sinks along with 2 portable toilets for potty trainers. There is one staff bathroom, which is separate. There is uncontaminated drinking water available to children both indoors and outdoors via individual labeled water bottles and water dispensers are available in each classroom for refills. The isolation area for a sick child will be established in the Director's office located near the front entrance. Preschool children will nap on cots and toddlers on mats. Bedding will be washed weekly. Children bring their own AM/PM snack and lunch, so no food will be prepared on site. There is emergency snacks if needed. The classrooms do contain age appropriate toys and activities available for children. Cubbies are available for each enrolled child. There is grass and wood chip cushioning material underneath the play structures sufficient to absorb falls. There are large fabric shade covers available to provide shade in the play yards. Toddlers and Preschoolers will have separate play yards. There are no bodies of water located on the property and none are to be added without prior notification and approval of the licensing agency. This report, as well as the Lead Exposure Testing Flyer, were reviewed and discussed with the applicant/director. All licensing reports are public information and must be made available upon request for at least three years. Based on the fire clearance and applicant request, the facility is approved for a capacity of 24 children. Any proposed changes to the physical plant, telephone number, or change of address shall be immediately reported to the Department Notice of Site Visit shall be posted for 30 days from today's visit. The following items must be submitted prior to licensure: 1. Proof that director is enrolled in the Preventative Health Practices Lead Training. A 90-day provisional license shall be granted upon submission of the above items. The provisional license cannot be extended. Prior to granting a regular license, the applicant shall provide proof of Preventative certification to complete the requirement for current Preventative Health Practices.
