What the inspector wrote

On the above date and time Licensing Program Analyst (LPA) Susan Brewer, arrived to conduct a Case Management Inspection in response to the receipt of a self-reported unusual incident report (UIR) from the facility. The LPA was greeted by Director Carla Reyes and granted entry to tour the facility inside and out. The UIR was received by the licensing agency on 10/27/2025. It indicates that on 10/22/2025 a subject child fell sustain an injury to their left pinky finger and received medical treatment as result of the injury. The LPA took a census of 41 children supervised by 8 staff. LPA S. Brewer, reviewed facility records, documents and interviews were conducted with pertinent parties which revealed that staff were present to supervise the daycare child when the incident took place. Interviews with pertinent parties stated children were playing indoors with wooden blocks. Pertinent parties described one child throwing a large wooden block to pile during clean-up time and landing on a subject child\342\200\231s left pinky finger. Pertinent parties stated medical attention was sought as result of the injury. The LPA inspected the play equipment in the indoor activity area. The LPA observed the play equipment of wooden blocks to be of different sizes. The LPA observed the equipment of wooden blocks to be age appropriate, in good condition with rounded edges. However, due to the weight of the solid wooden block, the impact on the subject child\342\200\231s finger from a distance, the child was injured. The LPA conducted a review of records which indicates there was adequate supervision of 6 staff supervising 29 children in 3 connected rooms. See LIC809C Page 2 - Case Management Incident It was determined that the incident was accidental and unintentional, during the normal course of play. The subject child sustained the injury to finger when to another child tossing or throwing the large wooden block to a pile during clean up. The facility followed their operational plan by notifying the authorized representatives of the injury. Since the incident, the subject child has returned to the daycare. Based on information gathered the facility acted appropriately by administering 1st aid to treat the subject child\342\200\231s injury. Therefore, no violations were determined at this time. No citations were issued on today's date. No Civil Penalties were issued on today's Date. An exit interview was conducted and a copy of this report was provided to Director Carla Reyes. A notice of Site Visit was given and must remain posted for 30 days.
