What the inspector wrote

On April 10, 2025, Licensing Program Analyst (LPA), Cindy Hamilton, conducted a Case Management during this visit, LPA Hamilton was provided LIC 9276 Child Care Center Sampling Checklist and LIC 9275 External Water Sampler Self-Certification, LIC 999 Facility Sketch and lead testing results dated May 8, 2023. The results provided from SWRCB, indicated that the facility had elevated levels of lead in water used for drinking and/or the preparation of food and the department was notified of the Action Level Exceedance (ALE). On May 4,2023, the sink identified as “B” Sink Faucet_Junior PreK Classroom tested at ALE 6.5 ppb. Per Ms. Betancourt the faucet was replaced approximately November 2024 due to having difficulty in finding a service provider to replace the faucet. The faucet was retested March 2025 and currently awaiting results. LPA advised Veronica Betancourt, teacher, that all water outlets tested with an ALE at the facility should be placed as out of service and children should be provided with an alternative potable water. Ms. Betancourt advised that the facility uses bottled water in the children’s own cups for indoor and outdoor consumption. The director will provide LPA with pictures of the faucet(s) originally tested, date sink/faucet was replaced and results from the March 28, 2025 retest. An exit interview was conducted with Veronica Betancourt-. A copy of this report was issued, along with a Notice of Site visit and appeal rights. This report shall be public record for three years.

    Based on record review, the director did not comply with the section cited above in the director did not have a current mandated reporter certificate on file. The certificate expired April 6, 2025 which poses/posed a potential health, safety or personal rights risk to persons in care.
