Inspector’s narrative

What the inspector wrote

On 05/25/2023 at 08:56AM, Licensing Program Analysts (LPAs) Perla Ordones and Samuel Lopez arrived at the facility for the purposes of concluding a Case Management inspection conducted on 02/23/2023 which was initially conducted in response to the receipt of an Unusual Incident Report (UIR) that was submitted by the facility on 02/14/2023. LPAs were granted entry into Room #2 by Facility Representative Laura Avalos-Sanchez. LPAs explained the purpose of today's inspection, completed a facility tour, and obtained a census. During the facility tour, LPAs observed 25 day-care children present at the facility. LPAs made observations, reviewed pertinent documentation, and conducted interviews with pertinent parties related to the self reported incident. It was reported that on 02/01/2023 a child was allegedly touched inappropriately by another day care child while in care. On or about 02/11/2023, the subject child\342\200\231s teacher received a text message from the subject child\342\200\231s mother requesting a call regarding an urgent matter. The subject child\342\200\231s mother stated the subject child disclosed they were touched inappropriately by another student while at the facility. On 02/13/2023, the subject child\342\200\231s mother informed the teacher she was dis-enrolling her child from the facility. During interviews, there was no evidence to corroborate the allegation that the subject child was touched inappropriately by another child while in care. Interviews disclosed that no one saw the incident occur and that the child who allegedly touched the subject child has "never" displayed that type of behavior, in the past. For these reasons, no violations have been found as of this time. Based on information obtained during follow up inspections, LPA Ordones determined there was no violations of Title 22 Regulations pertaining to the reported incident, at this time. An exit Interview was conducted and report was reviewed with Program Coordinator Linda Rodriguez. A Notice of Site visit was issued. No deficiency cited at this time.
