What the inspector wrote

On Wednesday, March 4, 2026 Licensing Program Analyst (LPA) Jesse Gardner conducted an unannounced Case Management in reference to information that was revealed during complaint number 10-CC-20260303093951. During the investigation in the complaint, it was revealed that the facility did not report an incident involving their air conditioning system that needed repair that affected three classrooms. The facility learned of this repair on February 11, 2026, and as of today’s date, has not reported the incident to Community Care Licensing per Title 22. Based on information gathered, a violation of Title 22 was identified and a citation was issued. An exit interview was held with Acting Director Jean Snook and a copy of this report was issued, along with copies of the LIC809-D, and a Notice of Site visit. This report shall be public record for three years.

    Based on record review, the licensee did not comply with the section cited above where S1 was found to not have a current Mandated Reporter certificate. This poses/posed a potential health, safety or personal rights risk to persons in care.

In addition, a written.. shall be submitted to the Department within seven days following the occurrence of such event.(1) Events reported.. following:(C) Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. This requirement was not being met as evidenced by: Based on interview, and LPA observation, the air conditioning to the Blue and Teal rooms was not functioning. This is a potential health and safety risk to children in care.
