What the inspector wrote

On June 5, 2025, at 10:31 AM, Licensing Program Analyst (LPA) Cindy Hamilton arrived at the facility to conduct a case management visit. The reason for the visit was to follow-up on an unusual incident report (UIR) submitted by the facility. At the time of visit, LPA toured the facility, took census, and met with Director \342\200\223 Kendra Woodcock to discuss the reported incident. The children involved in the incident were not interviewed by the LPA. The other UIR documented an incident on 05/23/2025, when a child was left unattended in the restroom. UIR stated a child was sent from playground to use the restroom in a classroom where there was a teacher present at the open door. The teacher present at the door went inside the classroom and the teacher was not aware the child had entered the classroom to use the restroom and then the teacher left classroom closing the door. Interviews revealed that the child was in the room for less than a minute before being discovered and the child was found to be safe and unharmed. The Director and staff were interviewed regarding the events. Based on information gathered, the facility acted appropriately and no violations have been identified. The facility notified parents and documentation of the incident was submitted to parent and Community Care Licensing (CCL) timely. An exit interview was held with Director \342\200\223 Kendra Woodcock and a copy of this report, appeal rights and Notice of Site visit was issued. Director was reminded that the notice of site visit must remain posted for 30 days. This report shall be public record for three years.
