What the inspector wrote

On June 5, 2025, at 9:10 AM, Licensing Program Analyst (LPA) Cindy Hamilton arrived at the facility to conduct a case management visit. The reason for the visit was to follow-up on an unusual incident report (UIR) submitted by the facility. At the time of visit, LPA toured the facility, took census, and met with Director \342\200\223 Kendra Woodcock to discuss the reported incident. The children involved in the incident were not interviewed by the LPA. The UIR documented an incident on 05/08/25, when a child was playing outside in the grass, looking for bugs and was stung by a bee. The sting caused the child\342\200\231s hand to swell, the stinger was removed, ice was applied and the child was monitored the rest of the day. The child was later taken to urgent care by the parent and treatment was prescribed. During the visit, LPA conducted interviews with staff and director and found that the staff involved did not report the incident to the director or to the child\342\200\231s parent. Based on the information gathered, facility failed to comply with Reporting Requirements per regulation section 101212 (f) , which specifies: (f) The items specified in (d)(1)(A) through (H) above shall also be reported to the child's authorized representative . See LIC809-D for cited deficiency. An exit interview was held with Director Kendra Woodcock and a copy of this report, appeal rights and Notice of Site visit was issued. Director was reminded that the notice of site visit must remain posted for 30 days. This report shall be public record for three years.
