What the inspector wrote

On December 17, 2025, at 10:00 am, Licensing Program Analyst (LPA) Cindy Hamilton conducted an unannounced case management visit at the facility and met with Director Kendra Woodcock. A case management visit is being conducted in response to the receipt of two Unusual Incident Reports (UIRs) from the facility. The first UIR was received by the licensing agency on December 3, 2025. This UIR indicates that on December 1, 2025, at approximately 11:11 am, Child #1 (C1) was left alone inside a classroom restroom. The second UIR was received by the licensing agency on December 12, 2025. This UIR indicates that on December 11, 2025, at approximately 3:40 PM, Child #2 (C2) was left inside of a classroom alone. These incidents occurred in two different classrooms. During the visit, there were no children present. Facility records and children\342\200\231s files were reviewed, and an interview was conducted with the director. Based on information gathered, the facility acted appropriately, and no violations have been identified. In both incidents, the children were quickly recovered from the classrooms without harm or injury and records indicate that the facility was operating within ratio. In addition, the facility notified parents of the incidents via incident report or verbally and also notified facility administration and Community Care Licensing (CCL) within the appropriate time frame. Director stated that the staff involved received disciplinary actions and additional training to review policy, procedures and expectations will be provided. Lastly, the Director will update the current procedure for transitioning to and from the classroom and will provide a copy to CCL upon completion. An exit interview was conducted and a copy of this report, appeal rights and Notice of Site Visit was provided to the Director. The Director was reminded that the Notice of Site Visit must remain posted for 30 days.
