Follow-up on corrections·October 9, 2025

Licensing Program Analyst's (LPA's) Hayley McCarthy and Tricia Danielson conducted an unannounced case management visit on October 9, 2025 at 09:30am to follow up on deficiencies from an annual inspection conducted on September 19, 2025. LPA met with director, Wendi Alcala who provided a tour of the preschool center. During the inspection, LPA's found that the train set on the outdoor playground had been sanded to prevent hazards to children in care. The gate surrounding the preschool that had sharp wire exposed was found to be repaired. The gate to the goat pin connected to the preschool playground had been repaired to ensure goat poop did not enter the playground. LPA's asked director to clean the remaining poop on the playground. The exposed rusted screws had been removed and the wall had been fixed. There were tight fitting lids on all trash cans. The hole under the building large enough for a child to crawl through had been repaired but has since broken. Director agreed to have maintenance fix the hole. The facility also informed LPA's that the director was no longer working at the facility and a new director was in place. Due to the new director not yet meeting all qualifications for a center director and the center not reporting the change to licensing within 10 days, a deficiency has been cited on the attached 809-D page. A notice of Site Visit was provided and must remain posted for 30 days. An exit interview was conducted, and this report was reviewed with Wendi Alcala. Appeal rights were discussed and provided during the exit interview.
