What the inspector wrote

On the above date and time Licensing Program Analyst (LPA) Susan Brewer, arrived to conduct a Case Management Inspection in response to the receipt of a self-reported unusual incident report (UIR) from the facility. The LPA was greeted by a staff member and granted entry to tour the facility inside and out. The Director Carla Reyes arrived at 8:13 AM to attend the inspection and the following was observed and/or discussed. No school children were present at the time of the inspection. The UIR was received by the licensing agency on 10/22/2025. It indicates that on 10/20/2025 a subject child fell during the course of outdoor play, sustaining an injury to their left arm and received medical treatment as result of the injury. LPA S. Brewer, reviewed facility records, documents and interviews were conducted with pertinent parties which revealed that staff were present to supervise the daycare child when the incident took place. Interviews with pertinent parties stated the subject child fell to the ground while attempting to avoid contact with a ball tossed between other children. The LPA inspected the location where the child fell in outdoor play area. The LPA observed the area of the fall to have rubber matting with padding to cushion a fall. The LPA observed the rubber matting to be in good condition and to have a bounce while walking. The LPA observed natural grass next to the rubber matting and play equipment intact, without protruding parts. It was determined that the incident was an accidental injury, during the normal course of play the subject child landed incorrectly when falling to the ground. The facility followed their operational plan by notifying the authorized representatives of the injury. Since the incident, the subject child has returned to the daycare. Based on information gathered, the facility acted appropriately, by administering 1st aid until medical professionals arrived to treat the subject child. No violations have been determined at this time. No citations or Civil Penalties were issued. An exit interview was conducted, and a copy of this report was provided to Director Carla Reyes. A notice of Site Visit was given and must remain posted for 30 days.
