On Thursday, March 12, 2026 at approximately 08:59AM, Licensing Program Analyst (LPA) Jesse Gardner conducted an unannounced Case Management inspection to follow-up on an Unusual Incident Report (UIR) submitted to Community Care Licensing CCL on March 2, 2026. LPA met with Director Nicole Fiorella and toured the facility. LPA interviewed the Director, and reviewed records, and made observations. On February 25, 2026, CCL received information via UIR that Child One (C1) left from the group of children during a transition from the playground to the classroom, and ended up leaving the perimeter of the school. C1 was caught by an administrator of the school. In addition, the incident was reported to CCL and documentation observed in the child\342\200\231s file and C1's parent was notified. Records review also confirmed that the CCC was operating within proper staff to child ratios. Based on information gathered, a violation of Title 22 has been identified, and the facility was issued a citation. An exit interview was held with Director Nicole Fiorella and a copy of this report was provided along with a copies of the LIC809-D, Notice of Site Visit, and Appeal Rights. This report shall be public record for three years.
