What the inspector wrote

Licensing Program Analyst (LPA) Samuel Lopez arrived at the facility to conduct a case management visit in response to the receipt of an Unusual Incident Report (UIR). The UIR was received by the licensing department on 4/16/2026. The UIR documented an incident that occurred on 4/15/2026, involving a child, that once they arrived at home, was observed with red marks on their arms, but the cause was unknown. Upon arrival this date on 4/29/2026, LPA Lopez met with Teacher Robin Zeldon and TOSA Sharon Bua and explained the purpose of the visit. Records were reviewed, documentation was obtained, and interviews were conducted. The following information was provided/obtained: On April 15, 2026, the child arrived at the facility, via a bus, and was upset and crying. Staff were able to calm the child down and conducted a health check and did not observe any concerns. Later that day, once the child was home, red marks were observed on the child's arms, according to the child’s parent/legal guardian. However, also according to child's parent/legal guardian, the child did not have any marks when leaving home the morning of 4/15/2026. On the day in question, 4/15/2026, the child was brought over from another child care facility near by and staff from that facility had struggled to get the child on the bus on time. LPA obtained information that the child may have had a hard time with saying good-bye and leaving the facility. Although the red mark were observed on the next day, April 16, 2026, they were gone by April 17, 2026. In reviewing facility records, there were no reports regarding any possible injury that may have occurred at the facility on the date in question. After the information regarding this incident was received and reviewed a determination could not be made as to when, where, or how the red marks may have occurred. Based on the information gathered, facility complied with reporting requirements, and other regulation requirements. No violations of Title 22 were identified during today’s inspection. A notice of site visit was given to TOSA Sharon Bua and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate citation. Exit interview conducted and report was reviewed with the TOSA Sharon Bua.
