What the inspector wrote

On date and time listed above, Licensing Program Analyst (LPA), Susan Brewer, arrived at the facility to conduct annual inspection. LPA was granted entry by Early Childhood Specialist (ECS) Cheryl Hood. LPA toured the facility inside and out, reviewed records, took a census and observed and/or discussed the following. Days & hours of operation: Monday-Friday, 8:00 AM to 11:00 AM, Preschool in Room 5 and 8:00 AM to 2:30 PM Head Start Program in Room 6. The inspection consisted of reviews of the CARE tool domains. The inspection found the facility to be in compliance, except as noted on the LIC809D page. If updates are made, the facility representative will submit the following documents to Licensing within 30 days: 1. LIC 500 Personnel Report: Update requested 2. LIC 610 Emergency & Disaster Plan: Update requested 3. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule. 4. LIC 309 Administrative Organization. 5. LIC 308 Designation of Administrative Responsibility: Update Requested 6. LIC200A application form with any changes The following items have been posted and are current: License; Emergency Disaster Plan (LIC610); Facility sketch; Earthquake Preparedness Checklist (LIC9148; Parent\342\200\231s Rights Poster (PUB393); Personal Rights (LIC613A); Child Car Seat Law, and Menu. The facility is operating within the scope of license with appropriate ratios and supervision during this inspection. The LPA took a census of 31 children supervised by 6 staff. Medications are stored inaccessible to children in the school nurse\342\200\231s office. If medication is stored in the classroom or refrigerator, the facility has a locked box available for storage. Hazardous and dangerous items are stored inaccessible to children. Poisons and toxins are locked in storage closet. Classrooms and floors are clean, free of hazards and rooms are equipped with age-appropriate furniture. Bathrooms were observed to be safe, sanitary and in operating condition. There are two toilets in the kitchen area, 1 sink in each classroom and additional sinks and toilets in the elementary playground area. Per Cheryl Hood no weapons are stored at the facility. There are no accessible bodies of water present on today\342\200\231s date. All wading pools or similar product must be emptied immediately after use and stored in an upright position. Outdoor activity areas are enclosed by appropriate fences, free of hazards and have age and size equipment in good condition. Areas around or under high climbing equipment have material that absorbs a fall. The LPA observed natural grass and rubber turf to cushion a fall. Two shade structures were observed in the outdoor play area. The LPA observed the exposed steel on the stairs and landing of the outdoor play structure due to the deterioration of protective coating. The coating was observed to be cracked, lifting and missing protective coating on several areas. The licensee representative stated there is a work order in process to repair the outdoor equipment. Food preparation area is clean, free of litter, and vermin. Food is stored appropriately and protected from contamination. Menus are current, posted and visible for authorized representatives and available upon request Measures are taken to keep the facility free of pests and rodents. Storage containers for solid waste have tight-fitting covers in good repair. Uncontaminated drinking water is readily available both indoors and outdoors: The facility has drinking water fountains both indoors and outdoors. In addition, children bring individual water bottles from home. Sign in/Sign out record was reviewed and meets regulation requirements: Authorized representatives use electronic sign in/out. A Staff member is present with CPR/First Aid which expires on 10/26/2026 Opening and closing staff member\342\200\231s CPR/First Aid expires on 10/26/2026 A fully qualified teacher completed Health and Safety Training on 03/11/2023 Children files are complete on today\342\200\231s date. Staff have received on-the-job training for housekeeping, sanitation and universal health precautions. Staff files are NOT complete on today\342\200\231s date; One staff present is missing proof of AB1207 renewal training. Fire & earthquake drills are conducted/documented every six months. Last drill on 02/13/2026 @ 10:45 AM. A fully charged fire extinguisher (2A:10BC) was observed and tagged by the fire department and needle in the green. A smoke detector and carbon monoxide detector were present and tested by the licensee during this inspection on 02/25/2026. Cheryl Hood was informed the Department has inspection authority per Health and Safety Codes. Cheryl Hood was reminded of the following: 1. Facility staff are responsible to know the regulations for providing care 4. Failure to meet the posting requirements shall result in an immediate civil penalty. 5. AB 1207 \342\200\223 Mandated Child Abuse Reporting: Child Day Care Personnel Training, beginning January 1, 2018 \342\200\223 Requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. AB2960 \342\200\223 Childcare and development services, online portal \342\200\223 Effective June 20, 2022 - This bill requires the State Superintendent of Public Instruction (SSPI), within the California Department of Education (CDE), to develop and post on CDE's website a comprehensive childcare and development services online portal for families and providers by June 30, 2022. Cheryl Hood was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. Cheryl Hood was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES. LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. LPA referred Cheryl Hood to the Department website for lead: Lead Toxicity Prevention and Water Testing Information. https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information . On 05/24/2024 completed lead testing for the water outlets used indoors and outdoors. On 08/30/2024 the agency, as well as the water outlets used in the school kitchen where meals are prepared. Reported results indicate not lead exceedances. Report on file. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. During the exit interview, the Cheryl Hood, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with ECS Cheryl Hood. The LPA observed the licensee representative post the Notice of Site Visit for public view, prior to exiting the facility.
