Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Jesse Gardner arrived unannounced to conduct a case management at the facility. Upon arrival, LPA met with Staff One (S1) Assistant Mariah Mercado who indicated there were 7 children in care. Staff Two (S2) Diana Sierra was also inside the facility. Licensee Judy Sadler was not initially present but arrived at the conclusion of the visit. During the visit, LPA toured the facility to verify the corrections and the following was noted. During the initial tour, LPA noted an infant (Child 1 - C1) in a crib with a blanket asleep. 1. For the deficiency cited on 05/01/2025, Licensee agreed to correct the bottom of the fencing measurement at no more than 2" surrounding the pool. On today's date, LPA noted not all areas to be in compliance exceed 2". New deficiency cited. 2. On 07/22/2025, LPA noted the Licensee had a telescoping 12' pole with body hook. On that date, LPA advised the Licensee the pole needed to be fixed, or "stationary". On today's date, LPA inspected the 12' pole, and determined that it is stationary. POC cleared. 3. For the deficiency cited on 07/22/2025, Licensee agreed to correct the height of the latch to above 60". On today's date, LPA noted correction to the latch, and that is in compliance with Title 22. Deficiency cleared. 4. For the deficiency cited on 05/01/2025, Licensee agreed to purchase a ASTM certified pool alarm. On today's date, LPA noted the alarm was in compliance. POC clear. 5. For the deficiency cited on 05/01/2025, Licensee agreed to purchase a US Coast Guard certified rescue life ring. On today's date, LPA noted the life ring was not noted to be in compliance. New deficiency cited. 6. For the deficiency cited on 05/01/2025, Licensee agreed to develop a pool inspection log. On today's date, LPA noted a pool log was being maintained. POC clear. 7. For the deficiency cited on 05/01/2025, Licensee agreed to move the chairs that would provide a child access to the pool. LPA noted the Licensee was in compliance. POC clear. 8. For the deficiency cited on 05/01/2025, Licensee agreed to remove the playground that was in need of repair. LPA noted the playground was removed. POC clear. An exit interview was conducted with Licensee Judy Sadler where a copy of this report was reviewed with and provided along with copies of the (2)LIC809-D (deficiency page) and copies of the Appeal Rights. A Notice of Site Visit was also provided and must be posted for 30 days.

This requirement was not being met as evidenced by: Based on interview with Licensee, and observation, LPA found that the Licensee was absent from the facility for more than 20 percent of the daily operation. This is a potential health and safety risk to children in care.

This requirement was not being met as evidenced by: Based on LPA's observation, the Licensee did not comply with a gap that is no more than 2 inches at the bottom of the fence surrounding the pool. This is a potential health and safety and/or personal rights risk to children in care.

This requirement was not being met as evidenced by: Based on LPA observation, and staff interview, a life ring approved by the US Coast Guard was not available at the facility. This is a potential health and safety and/or personal rights risk to children in care.
