What the inspector wrote

On 12/04/2025, Licensing Program Analyst (LPA) Jesse Gardner conducted an unannounced Case Management visit to the facility to follow up on deficiencies that were cited on 09/17/2025. Present when LPA arrived was the Licensee and both of their assistants and 2 children. During the initial visit on 09/17/2025, Upon the tour of the inside of the facility, the Licensee was not present; however, arrived well into LPA's visit. Licensee explained they would provide a plan to account for their absences, and provide proof that they understood the regulation where they can not be absent from the facility for more than 20 percent of the day. Further, LPA found an infant in a crib with a blanket, and the Licensee agreed to provide infant safe sleep training to staff to LPA by 09/24/2025. Additionally, LPA then toured the outside of the facility and in the pool area of the backyard, LPA found a gap at the bottom of the fence that exceeded 2". LPA also noted the life ring that the Licensee had, was not US Coast Guard approved and Licensee stated they would provide proof of a new one by 09/24/2025. On today's visit, LPA found the following: 1. LPA noted all areas in the pool area to be in compliance not to exceed 2". 2. The life ring meets US Coast Guard standard, and is at least 17" in diameter 3. Licensee provided proof of training for infant safe sleep for their staff 4. Licensee provided a statement of understanding to comply with absences. Additionally, Licensee Judy Sadler advised LPA they have a new enrolled child (C1). LPA asked to see the file of the new child and found missing items. Deficiencies that were cited on 09/17/2025 are cleared. On today's date, deficiencies were cited on LIC809-D. An exit interview was conducted with Licensee Judy Sadler and a copy of this report was reviewed with and provided along with a copy of the Appeal Rights. A Notice of Site Visit was also provided and must be posted for 30 days.
