Inspector’s narrative

What the inspector wrote

A audio recording of the licensee providing care to the children was obtained but it could not be confirmed if the sounds in the audio were the licensee slamming the child into any chair or slapping the child. Although the allegations may have happened or are valid, there is not a preponderance of evidence to proof the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED. Exit interview was conducted. Appeal rights were discussed and provided along with a copy of this report to the Licensee on this date. A Notice of Site Visit was issued and shall remain posted for 30 days. The investigation revealed the following: It was alleged that the licensee had yelled at a child in care and told the child they would not get there food if they continued crying. The department obtained audio recording of the licensee screaming at the child and telling the child "stop and sit down before I don't give you any breakfast". Based on the evidence obtained this allegation is substantiated. See LIC9099-D for deficiency cited. A Notice of Site Visit was posted. An exit interview was conducted, appeal rights discussed and provided along with a copy of this report to Licensee on this date.

This poses a potential health, safety or personal rights risk to persons in care.
