Based on observation, the licensee did not comply with the section cited above where a play tunnel was observed to have the potential to snag a child causing injury. Additionally, observation of rusted bikes, and broken play equipment was observed. This which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above where, upon inspection of the backyard, LPA discovered loose fitted wood panel that had exposed, and protruding nails, and screws. This poses/posed a potential health, safety or personal rights risk to persons in care.
