What the inspector wrote

On Wednesday, July 30, 2024 Licensing Program Analyst (LPA) Jesse Gardner conducted an unannounced Case Management inspection to follow-up on an Unusual Incident Report (UIR) submitted to Community Care Licensing CCL on July 16, 2025. LPA met with Assistant Director Patricia Gibson and conducted a tour a facility. On July 16, 2025, CCL received information via UIR that a Staff (S1) tapped Child One (C1) on the head with the end of a broom. No injury was noted as a result of the incident. Personal Rights training with all staff was conducted on July 17, 2025. LPA received a copy of the training roster. The incident was reported to CCL and documentation observed in the child’s file and parents were notified. Based on information gathered, the CCC acted appropriately and no violations of Title 22 have been identified. An exit interview was held with Patricia Gibson. A copy of this report was issued, along with a Notice of Site visit. This report shall be public record for three years.
