What the inspector wrote

On 05/16/2025 at 9:31 AM, Licensing Program Analyst (LPA) Brian Morris conducted an unannounced Case Management inspection to follow-up on an Unusual Incident Report (UIR) submitted to Community Care Licensing on 04/17/2025. LPA met with Director Misty Kelley and toured the facility and gathered census at 9:40 AM. During this visit, LPA interviewed site supervisor, toured facility, took census and gathered additional information regarding the incident. Per incident reports, on April 17, 2025 at approximately 12:05 PM, students were transitioning from outdoor play into the restroom located in the classroom #41 for handwashing \342\200\223 a half door divides the classroom and restroom area. There were 16 students present with two staff. The lead teacher was not in the classroom on 04/17/2025, due to completing on site training in Classroom area #45. The substitute teacher was in the classroom area with a group of students, helping transition to lunch and the CDA was with a group of students in the bathroom helping was their hands. While the CDA was in the restroom with the students, she heard two students arguing/crying/fighting in the classroom area and without thinking, left the restroom area to assist those children, leaving C1 in the restroom area alone for approximately three minutes, before the student was located by the lead teacher. Lead teacher comforted the child, notified C1\342\200\231s mother. C1\342\200\231s mother asked C1, why didn\342\200\231t they open the door. C1 responded, they did not know why, lead teacher notified administrators of the incident. C1's parents were contacted and notified of the incident. C1 remained on site and completed a full day on 04/17/2025, per Director Kelley. In addition, the incident was reported to CCL. Records review also confirmed that the CCC was operating within proper staff to child ratios. Additionally, LPA observed the restroom area of incident, and found no Title 22 violations that would have contributed to the incident. See LIC 809-D for deficiency cited. An exit interview was held with Director Misty Kelley. A copy of this report was issued, along with a Notice of Site visit. This report shall be public record for three years.
