What the inspector wrote

On November 05, 2025, at 12:05 PM Licensing Program Analyst (LPA) Brian Morris arrived at Armada Elementary Head Start Program #334805395 to conduct a case management visit to address UIR follow up for incident dated 10/27/2025. This LPA requested documentation, interviewed Program Coordinator Anita Hernandez. During the facility tour, this LPA toured the pick-up/drop off area, classroom #40, classroom #41, classroom #44 and classroom #45. On 10/27/2025 it was alleged that two parents picking up their child observed a Child development assistant teacher forcefully pushing a child\342\200\231s shoulders to have them sit down. Child and Teacher unknown. Program Coordinator reports, We contract our Child development assistants teachers from Child Care Careers (CCC), on 10/27/2025, two parents reported they observed a Child development assistant teacher forcefully pushing an Unknown child into a seated position. Program Coordinator reports, we did discover the Child development assistant teacher as a substitute staff (CCC worker) in question, and they have been removed from the Moreno Valley Unified School District (MVUSD) Head Start Program teaching list and will not be allowed to substitute at any MVUSD facility. Program Coordinator interviewed two full time teachers, who were in the same classroom on 10/27/2025, both teachers deny seeing or observing any concerning incidents from the Child development assistant teacher, due to being busy working with other children in care. Program Coordinator reports, we will be having a Refresher for all CCC staff who are substitute teachers with MVUSD. Program Coordinator reports, the refresher group will consist of 10-15 employees per session, and the refresher training will be an hour long. This LPA will follow up with Program Coordinator Anita Hernandez for additional information. This LPA has no current concerns for the facility at the moment. An exit interview was conducted, and a copy of this report was provided to the Program Coordinator Anita Hernandez and Program Coordinator Star Monville. A Notice of Site Visit was provided, and Program Coordinator understands this must remain posted for 30 days.
