What the inspector wrote

On Wednesday, May 14, 2025 Licensing Program Analyst (LPA) Jesse Gardner conducted an unannounced Case Management inspection to follow-up on an Unusual Incident Report (UIR) submitted to Community Care Licensing (CCL) on May 1, 2025. LPA met with Director Cathy Knudsen and conducted a tour of the facility. LPA interviewed staff, and gathered documents for review during this inspection. On May 1, 2025, CCL received information via UIR that on April 29, 2025, Child One (C1) sustained a puncture wound on the left side of their forehead. First aide was applied, and their parent was immediately notified. C1 was then taken to see a physician. Staff interviews revealed to LPA that proper and timely aid was provided to the injured child. In addition, the incident was reported to CCL and documentation observed in the injured child\342\200\231s file and C1's parent was notified. Records review confirmed that the CCC was operating within proper staff to child ratios. At the time of this report, CCL needs to gather further information to be able to determine an outcome in this matter. An exit interview was held with Director Cathy Knudsen. A copy of this report was issued, along with a Notice of Site visit. This report shall be public record for three years.
