Licensing Program Analyst (LPA) Samuel Lopez arrived at the facility to conduct a case management visit in response to the receipt of an unusual incident report (UIR). The UIR was received by the licensing agency on 9/12/2025. The UIR documented management being informed on 9/10/2025 of the possible personal rights violations, which included withholding food, shaming, leaving children in soiled clothes, and grabbing children by their arms. Dates and times of when this may have occurred were not provided. Upon arrival, this date on 10/16/2025, LPA Lopez met with facility ECE Coordinator Linda Rodriguez and stated the purpose of the visit. Records were reviewed and interviews were conducted with staff and children that were in attendance during the inspection. LPA Lopez obtained additional information regarding the incident. This information included details that a staff/teacher would not provide milk or water to child(ren) due to their behavior and at times being told that they were not smart. Staff/teacher would blame children for being left soiled because they did not speak up. Also, staff/teacher would raise their voice at child(ren) and placing blame on them for doing so. Please note that staff denied any and all allegations listed above. Based on the information gathered, facility failed to comply with the requirement to ensure that all children in care are accorded their Personal Rights. See LIC809-D for cited deficiency. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Also, LPA Samuel Lopez informed the ECE Coordinator Linda Rodriguez to provide a copy of this licensing report dated October 16, 2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
