Inspector’s narrative

What the inspector wrote

standing, the teacher let one arm go and the child fell backward and hit child\342\200\231s head on the floor. Confidential interview also disclosed that infant was being held by one arm as the staff attempted to assist another child with staff's other hand, resulting in the infant falling from staff's one-handed grasp to the floor. Confidential interviews also disclosed to LPA that the incident was an accident and no disciplinary action was taken. Confidential interviews revealed that the CCC's cameras were reviewed by Director and staff. LPA was unable to clarify how child fell from staff\342\200\231s hold but confirmed that the child was dropped on at least one occasion. Based on confidential interviews and records review, the preponderance of evidence has been met and the allegation that staff dropped an infant in care is substantiated. The facility is being cited for Title 22, Section 101223 (a)(2) Personal Rights which poses an immediate health, safety and/or personal rights risk to children in care. An exit interview was conducted, a copy of this report and appeal rights were provided to Director. The Director was reminded that the Notice of Site visit must remain posted for 30 consecutive days.
