On October 3, 2025, at 8:00 AM, Licensing Program Analyst (LPA) William Chancellor arrived unannounced at Childtime Children's Center (CCC) and was greeted by Assistant Director (AD) Antionette Andrews. A case management visit is being conducted in response to the receipt of an Unusual Incident Report (UIR) received by Community Care Licensing (CCL) on September 26, 2025. AD self reported an incident involving kindergarten age children being transported without appropriate restraint devices. The UIR was reviewed and confidential interviews were conducted with two staff (S1-S2). Documents relevant to the incident were obtained and LPA confirmed that the facility was within ratio at the time of the incident. Interviews and records review confirm that between September 18-19, 2025, five or more different children were transported in a rented vehicle without appropriate booster seats. Two of two interviews also confirmed that the CCC transportation buses have built in booster seats that are not utilized when transporting kindergarten aged children. Based on information gathered, the facility is being cited under Title 22: 101225 Transportation. Please refer to 809-D for cited deficiency. An exit interview was conducted, and a copy of this report was provided to Assistant Director, Antionette Andrews. A Notice of Site Visit was provided and must remain posted for 30 consecutive days.
