What the inspector wrote

On Wednesday, February 4, 2026 Licensing Program Analyst (LPA) Jesse Gardner conducted an unannounced Case Management inspection to follow-up on an Unusual Incident Report (UIR) submitted to Community Care Licensing CCL on December 5, 2025. LPA met with Director Maria Marquez and toured the facility and the area of incident. LPA interviewed the Director, involved staff, and reviewed documents. On December 5, 2025, CCL received information via UIR that on December 3, 2025, Child One (C1) obtained an injury while at the center. Staff notified C1's parent that C1's finger had been caught in the playground gate, and was observed by staff. C1 was given basic first-aid. It was later revealed after a visit to the doctor that C1 had sustained a minor fracture to their finger. On today's date, LPA determined staff met ratio requirements on the day of incident via record review and interview, and observed that the playground gate is in normal functioning order without any areas of concern. In addition, since the incident, the facility has placed a plastic guard to prevent children's fingers from getting in-between the gate and fence. Based on information gathered, the CCC acted appropriately and no violations of Title 22 have been identified. An exit interview was held with Director Maria Marquez. A copy of this report was issued, along with a Notice of Site visit. This report shall be public record for three years.
