What the inspector wrote

On Wednesday, December 17, 2025 Licensing Program Analyst (LPA) Jesse Gardner conducted an unannounced Case Management inspection in relation to information revealed during the investigation into complaint number 10-CC-20250706231609. During the interview process with staff, and the Director, and a review of documentation received, it was determined that C1's parent was not notified on the day of incident when C1 presented a possible injury during class. Based on information gathered, the CCC did not act appropriately and a violation of Title 22 was identified. An exit interview was held with Director Jesenia Garcia Macia along with a copy of the LIC809-D, and Appeal Rights. A copy of this report was issued, along with a Notice of Site visit. This report shall be public record for three years.

The licensee shall obtain specific instructions from the authorized representative regarding action to be taken.(2) In the case of less serious injuries including, but not limited to, minor cuts, scratches and bites from other children requiring assessment and/or administration of first aid by staff, the licensee shall document the injury in the child's record and notify the child's authorized representative of the nature of the injury when the child is picked up from the center. This was not being conducted as evidenced by: Based on interviews with staff and the Director, it was determined that C1's parent. This is a potential health and safety and/or personal rights risk to children in care.
