Facility-requested visit·February 11, 2022

Licensing Program Analyst (LPA), Rachel Zeron was an announced visit to approve a new playground structure. LPA Arrived at the facility and met with the Director and Assistant Director. LPA toured the facility, both inside and outside and took census. LPA inspected the playground area which located on the Northwest side of the building. This updated structure replaces be original play structure that was removed. LPA inspected the structure and found the structure to be safe for children in care. There are wood chips on the floor area of the playground area. LPA is approving the new play structure for day-care children to use. No deficiencies were cited. LPA conducted an exit interview with the Director and Assistant Director and a copy of this report was issued. A Notice of Site visit was issued and must be posted for the next 30 days
