What the inspector wrote

Based on record review and LPA observation, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. See LIC 9099D An exit interview was conducted, and this report was reviewed with the licensees Concepcion and Rigoberto Marentes. Appeal rights were discussed and provided during the exit interview. A notice of site visit was given and must remain posted for 30 days.

This requirement was not met:Based on record review and observation, facility was operating with 13 children, 4 under the age of 2. On date of LPA visit, 13 children, 4 under the age of 2, with only one adult. This poses an immediate risk to the health, safety and personal rights of children.
